Instructions

Each person must fill out an individual application form. It is important that you print clearly. Adult membership is free for all residents of the Bay of Plenty.  Children's cards are issued free but the application form must be signed by a responsible adult.

Two forms of ID need to be shown:

1 x photo ID eg. passport, licence
1 x name and address eg. bank statement, phone bill

  • Accepts responsibility for the use of the library card.
  • Agrees to show the library card when borrowing from any district library.
  • Agrees to abide by Library policies adopted by Whakatane District Council.
  • Accepts responsibility for Library materials issued on the library card.

    The information contained on the application form is for the purposes of effective management of the Library only.

    The information shall not be used for any other purpose nor released to any other person without your specific authorisation or unless so authorised by current legislation.

    Should you at any time wish to review the information collected by this form and held by the Library and/or to correct such information you are entitled to do so on proof of your identity.
